Book of Remembrance 5786

Use this form to reserve dedications and listings for the YIJE 5786 Book of Remembrance and Yizkor Service. This edition will be used for all Yizkor services in 5786.

Dedications will be listed under your family name. Individual memorialized names can be listed for $18 each. Alternatively, an unlimited number of memorialized names can be listed for $180. In all cases, the name of the departed individual will be inscribed in both Hebrew and English. Please note there is an option below to simply repeat your dedications from last year.

All names already memorialized with plaques on the YIJE Memorial Boards will be listed in this book at no charge. If you would like these names included with the rest of your family listings, simply include them with your other names on this form at $18 per name ($180 unlimited names).

Special dedications are also available using the form below.

Submissions must be made by Friday, September 12th so that the booklet can be printed in time for Yom Kippur.
